ABSTRACT

OBJECTIVES: This study was to examine the changes in dietary habits and food and nutrient intakes between before (2019) and during (2020) the coronavirus disease 2019 (COVID-19) pandemic from the Korea National Health and Nutrition Examination Survey (KNHANES). METHODS: A total of 54,995 participants aged ≥19 years who participated in the 2011-2020 KNHANES were included. The 10-year trend (2011-2020) and differences between 2019 and 2020 for dietary habits and food and nutrient intakes were estimated using SAS. RESULTS: In the past 10 years (2011-2020), the dietary habits (increase in skipping meals and eating out), food intake (increase in meats and decrease in fruits and vegetables), and nutrient intake (increase in fat and decrease in sodium) in adults have changed. When comparing between 2019 and 2020, there were 4.6%p decrease in the eating out more than once a day. On the other hand, there were no significant differences in the intakes of food, energy and most of nutrients between 2019 and 2020, except for the proportion of energy intake from fat (1.0%p increase) and carbohydrate (1.0%p decrease). CONCLUSIONS: Although a change in dietary habits from before to during the COVID-19 pandemic was observed, food and nutrient intakes have not deteriorated markedly and appear similar to the trends in the past 10 years. Throughout the COVID-19 pandemic, it is necessary to monitor the effects of changes in dietary habits on health as well as food and nutrient intakes.

ABSTRACT

Confusing health messages and environmental changes intended to prevent the spread of the COVID-19 virus have affected the dietary behavior of older African Americans. We investigated the impact of COVID-19-related factors on diet quality and the relationship between food access and diet quality. We surveyed 150 African Americans aged 55 years and above during the COVID-19 pandemic. The data obtained included socio-demographic and health information, and COVID-19-related knowledge and perceptions. Dietary intake data was obtained using the Diet History Questionnaire III. Analyses included bivariate and multivariable statistics. Overall, based on United States Department of Agriculture guidelines, the diet quality of older African Americans was poor. Lower knowledge and a lower perceived threat of COVID-19 were significantly associated with poor diet quality. Additionally, older African Americans with chronic diseases and food insecurity had poor diet quality. The COVID-19 pandemic has highlighted the fragility of diet quality. The combined impact of poor knowledge and perceived threat of COVID-19, chronic disease, and food insecurity contribute to poor diet quality in this population. This study adds to the well-known need for strategies to support the right to a healthy diet, particularly during COVID-19 and future pandemics. Proactive interventions to counteract the potential consequences of poor diets are needed.

ABSTRACT

Importance: Information about the trend in illicit substance ingestions among young children during the pandemic is limited. Objectives: To assess immediate and sustained changes in overall illicit substance ingestion rates among children younger than 6 years before and during the COVID-19 pandemic and to examine changes by substance type (amphetamines, benzodiazepines, cannabis, cocaine, ethanol, and opioids) while controlling for differing statewide medicinal and recreational cannabis legalization policies. Design, Setting, and Participants: Retrospective cross-sectional study using an interrupted time series at 46 tertiary care children's hospitals within the Pediatric Health Information System (PHIS). Participants were children younger than 6 years who presented to a PHIS hospital for an illicit substance(s) ingestion between January 1, 2017, and December 31, 2021. Data were analyzed in February 2023. Exposure: Absence or presence of the COVID-19 pandemic. Main Outcome(s) and Measure(s): The primary outcome was the monthly rate of encounters for illicit substance ingestions among children younger than 6 years defined by International Statistical Classification of Diseases, Tenth Revision, Clinical Modification diagnosis code(s) for poisoning by amphetamines, benzodiazepines, cannabis, cocaine, ethanol, and opioids. The secondary outcomes were the monthly rate of encounters for individual substances. Results: Among 7659 children presenting with ingestions, the mean (SD) age was 2.2 (1.3) years and 5825 (76.0%) were Medicaid insured/self-pay. There was a 25.6% (95% CI, 13.2%-39.4%) immediate increase in overall ingestions at the onset of the pandemic compared with the prepandemic period, which was attributed to cannabis, opioid, and ethanol ingestions. There was a 1.8% (95% CI, 1.1%-2.4%) sustained monthly relative increase compared with prepandemic trends in overall ingestions which was due to opioids. There was no association between medicinal or recreational cannabis legalization and the rate of cannabis ingestion encounters. Conclusions and Relevance: In this study of illicit substance ingestions in young children before and during the COVID-19 pandemic, there was an immediate and sustained increase in illicit substance ingestions during the pandemic. Additional studies are needed to contextualize these findings in the setting of pandemic-related stress and to identify interventions to prevent ingestions in face of such stress, such as improved parental mental health and substance treatment services, accessible childcare, and increased substance storage education.

ABSTRACT

BACKGROUND: Diet, a key component of type 1 diabetes (T1D) management, modulates the intestinal microbiota and its metabolically active byproducts-including SCFA-through fermentation of dietary carbohydrates such as fiber. However, the diet-microbiome relationship remains largely unexplored in longstanding T1D. OBJECTIVES: We evaluated whether increased carbohydrate intake, including fiber, is associated with increased SCFA-producing gut microbes, SCFA, and intestinal microbial diversity among young adults with longstanding T1D and overweight or obesity. METHODS: Young adult men and women with T1D for ≥1 y, aged 19-30 y, and BMI of 27.0-39.9 kg/m2 at baseline provided stool samples at baseline and 3, 6, and 9 mo of a randomized dietary weight loss trial. Diet was assessed by 1-2 24-h recalls. The abundance of SCFA-producing microbes was measured using 16S rRNA gene sequencing. GC-MS measured fecal SCFA (acetate, butyrate, propionate, and total) concentrations. Adjusted and Bonferroni-corrected generalized estimating equations modeled associations of dietary fiber (total, soluble, and pectins) and carbohydrate (available carbohydrate, and fructose) with microbiome-related outcomes. Primary analyses were restricted to data collected before COVID-19 interruptions. RESULTS: Fiber (total and soluble) and carbohydrates (available and fructose) were positively associated with total SCFA and acetate concentrations (n = 40 participants, 52 visits). Each 10 g/d of total and soluble fiber intake was associated with an additional 8.8 µmol/g (95% CI: 4.5, 12.8 µmol/g; P = 0.006) and 24.0 µmol/g (95% CI: 12.9, 35.1 µmol/g; P = 0.003) of fecal acetate, respectively. Available carbohydrate intake was positively associated with SCFA producers Roseburia and Ruminococcus gnavus. All diet variables except pectin were inversely associated with normalized abundance of Bacteroides and Alistipes. Fructose was inversely associated with Akkermansia abundance. CONCLUSIONS: In young adults with longstanding T1D, fiber and carbohydrate intake were associated positively with fecal SCFA but had variable associations with SCFA-producing gut microbes. Controlled feeding studies should determine whether gut microbes and SCFA can be directly manipulated in T1D.

BACKGROUND: The Coronavirus disease 2019 (COVID-19) pandemic has had a devastating impact on health systems, food supplies, and population health. This is the first study to examine the association between zinc and vitamin C intakes and the risk of disease severity and symptoms among COVID-19 patients. METHODS: This cross-sectional study included 250 recovered COVID-19 patients aged 18-65 years from June to September 2021. Data on demographics, anthropometrics, medical history, and disease severity and symptoms were collected. Dietary intake was evaluated using a web-based, 168-item food frequency questionnaire (FFQ). The severity of the disease was determined using the most recent version of the NIH COVID-19 Treatment Guidelines. Using multivariable binary logistic regression, the association between zinc and vitamin C intakes and the risk of disease severity and symptoms in COVID-19 patients was evaluated. RESULTS: The mean age of participants in this study was 44.1 ± 12.1, 52.4% of them were female, and 46% had a severe form of the disease. Participants with higher zinc intakes had lower levels of inflammatory cytokines, such as C-reactive protein (CRP) (13.6 vs. 25.8 mg/l) and erythrocyte sedimentation rate (ESR) (15.9 vs. 29.3). In a fully adjusted model, a higher zinc intake was also associated with a lower risk of severe disease (OR: 0.43; 95% CI: 0.21, 0.90, P-trend = 0.03). Similarly, participants with higher vitamin C intakes had lower CRP (10.3 vs. 31.5 mg/l) and ESR serum concentrations (15.6 Vs. 35.6) and lower odds of severe disease after controlling for potential covariates (OR: 0.31; 95% CI: 0.14, 0.65, P-trend = <0.01). Furthermore, an inverse association was found between dietary zinc intake and COVID-19 symptoms, such as dyspnea, cough, weakness, nausea and vomiting, and sore throat. Higher vitamin C intake was associated with a lower risk of dyspnea, cough, fever, chills, weakness, myalgia, nausea and vomiting, and sore throat. CONCLUSION: In the current study, higher zinc and vitamin C intakes were associated with decreased odds of developing severe COVID-19 and its common symptoms.

ABSTRACT

This study aimed to investigate whether oral health behaviors were related to the dietary intake of vitamins. In this cross-sectional study, we included respondents of the 2016 national health and nutrition examination survey, and dental diseases from Hyogo Prefecture, Japan. Data on sociodemographic characteristics, findings of blood tests related to metabolic syndrome, dietary intake, oral health status, and behaviors were collected. Participants were divided into two groups based on their oral health behavior: the yes group (performed interdental cleaning or tongue brushing) and the no group (did not perform the behaviors). The study included 218 participants (male: 107, female: 111) aged 64.5 (range, 22-93) years. There were 133 (61.0%) and 85 (39.0%) participants in the yes and no groups, respectively. The daily intake of vitamins A, B2, B6, E, and K, folic acid, and niacin in the yes group was significantly higher than that in the no group. Oral health behavior correlated with the intake of vitamin B2 (p = 0.029), folic acid (p = 0.006), and vitamin K (p = 0.043) after adjusting for possible confounders. Oral health behavior (interdental cleaning or tongue brushing) correlated with the daily intake of vitamins B2, K, and folic acid.

OBJECTIVES: The COVID-19 pandemic has brought a significant impact on people's behaviour and lifestyles. Limited research has been conducted on the impact of COVID-19 on Malaysian university students' lifestyle modifications. This study aims to identify the impact of COVID-19 on dietary intake, sleeping patterns and physical activity levels among Malaysian university students. METHODS: A total of 261 university students were recruited. Sociodemographic and anthropometric data were collected. Dietary intake was assessed using PLifeCOVID-19 questionnaire, sleep quality was assessed using Pittsburgh Sleep Quality Index Questionnaire (PSQI) while physical activity level was assessed using International Physical Activity Questionnaire-Short Forms (IPAQ-SF). SPSS was used to perform statistical analysis. RESULTS: 30.7% of the participants adhered to the unhealthy dietary pattern, 48.7% had poor quality of sleep and 59.4% engaged in low physical activity levels during the pandemic. Unhealthy dietary pattern was significantly associated with a lower IPAQ category (p=0.013), and increased time spent sitting (p=0.027) during the pandemic. Participants being underweight before the pandemic (aOR=2.472, 95% CI=1.358-4.499), increased takeaway meal consumption (aOR=1.899, 95% CI=1.042-3.461), increased snacking between meals consumption (aOR=2.989, 95% CI=1.653-5.404) and engaged in a low level of physical activity during pandemic (aOR=1.935, 95% CI=1.028-3.643) were the predictors of unhealthy dietary pattern. CONCLUSIONS: The university students' dietary intake, sleeping patterns, and physical activity levels were impacted in different ways during the pandemic. Strategies and interventions should be developed and implemented to improve the dietary intake and lifestyle of the students.

ABSTRACT

The purpose of this study was to determine the stress, anxiety and eating conditions of health personnel working in COVID-19 clinics, to evaluate how they interact with each other with a holistic approach, and to examine the role of variables such as gender and BMI in these relationships. It was determined that 1-unit increase in the TFEQ-18 score decreased stress and anxiety levels 1.09 and 1.028 times, respectively. We were able to show that stress and anxiety levels of participants negatively affect their eating behavior and anxiety levels of health personnel negatively affect their eating behavior.

ABSTRACT

Selenium (Se) is an essential trace element which has an important role as a constituent of seleno-proteins involved in various physiological processes. Previous research in Irish adults suggests that intakes of this important nutrient are suboptimal. The aim of the present study was to estimate the current intakes and major food sources of Se by Irish adults. Mean daily intakes (MDIs) of Se were calculated using data from the National Adult Nutrition Survey which involved 1500 Irish adults aged 18-90 years. The Se content of foods and drinks consumed over a 4-d period was determined using data from the Irish Total Diet Study (TDS). Adequacy of Se intakes was assessed by calculating the proportion of the population with intakes below the adequate intake (AI) of 70 µg/d and lower reference nutrient intake of 40 µg/d (LRNI). The MDI of Se in the total population was 71⋅7 µg/d, with significantly higher intakes reported in men (80⋅2 µg/d) compared with women (63⋅4 µg/d, P < 0⋅01). Meat and meat products were the major contributing food group to Se intakes for both men (37 %) and women (31 %). Overall, 47 % of the population were not meeting the recommended AI, while 4 % of the total population were not meeting the LRNI. Although the average intake of Se is above the AI, a significant proportion of the population is not meeting this recommendation and continued monitoring of Se intakes is necessary, particularly by at-risk groups and also in the context of sustainability.

Objective: To examine the effects of macro- and micro-nutrient intake of health workers on hedonic hunger. METHODS: The descriptive, cross-sectional study was conducted at Kahramanmaras Necip Fazil City Hospital, Turkey, from May to December 2021, and comprised all types of healthcare professionals of either gender aged >18 years. Data was collected using a 22-question survey form to record three-day food consumption, and the Power of Food Scale. Data was analysed using SPSS 22. RESULTS: Of the 516 participants, 255(49.4%) were males and 261(50.6%) were females. The overall mean age was 41.28±7.598 years. Body mass indiex was the only factor significantly associated with hedonic hunger (p<0.05), while gender, age, meal-skipping status, the most skipped meal, and the occupational status had no such association (p>0.05). Nurses consumed high-energy macronutrients (p<0.05). CONCLUSIONS: Overweight health professionals were found to have the highest rate of hedonic hunger, while nurses' consumption of high-energy macronutrients was signficantly higher.

BACKGROUND AND AIM: While daily witnessed opioid agonist treatment (OAT) ingestion is common in British Columbia (BC), Canada, and elsewhere, sparse evidence supports this resource-intensive practice. Many settings across North America relaxed restrictions for take-home dosing during the COVID-19 pandemic and have reported consistent or improved patient outcomes. This study measured excess expenditures attributed to daily witnessed pharmacy dispensing compared with weekly or biweekly dispensation schedules. DESIGN, SETTING AND PARTICIPANTS: This study was a population-level retrospective analysis. We included all methadone, buprenorphine/naloxone and slow-release oral morphine dispensations in BC from 1 January 2014 to 30 December 2020. A total of 24 357 107 OAT dispensations among 51 195 unique individuals with 122 793 person-years of follow-up were included during the study period. MEASUREMENTS: Total expenditures for each person-week of OAT with an estimated expenditure under two scenarios are as follows: (1) a weekly dispensation scenario and (2) a biweekly dispensation scenario. FINDINGS: We estimated excess expenditures attributable to current dispensing practices of between $38 million (2014) and $47.4 million (2018) compared with a hypothetical weekly dispensing schedule, and $43.9 million (2014) to $54.9 million (2018) compared with biweekly dispensing. The majority of these expenditures (58-64%) were attributed to pharmacy dispensing fees ($23 million in 2014 to $30 million in 2018 compared with weekly dispensing; $26.6 million in 2014 to $34.7 million in 2018 compared with biweekly dispensing). CONCLUSION: Daily witnessed opioid agonist treatment ingestion results in more than $30 million in excess expenditures annually in the province of British Columbia, Canada compared with the costs of weekly or biweekly dispensation schedules.

ABSTRACT

Psychological distress is linked to unhealthy eating behaviors such as emotional eating and consumption of high-sugar food and drinks. Cross-sectional studies from early in the COVID-19 pandemic showed a high occurrence of worries and psychological distress, and this was associated with emotional eating. Few larger studies have examined how this coping pattern develops over time. This cohort study with 24,968 participants assessed changes over time in emotional eating, consumption of sugary foods as an example of unhealthy food choices, and consumption of fruits and vegetables as an example of healthy food choices. Further, associations between these and psychological distress, worries, and socio-demographic factors were assessed. Data were collected at three time points (April 2020, initially in the COVID-19 pandemic, then one and two years later). Emotional eating and intake of sugary foods and drinks were high at the start of the pandemic, followed by a reduction over time. High psychological distress was strongly associated with higher levels of emotional eating and high-sugar food intake, and lower levels of healthy eating habits. The strength of this association reduced over time. Our findings indicate the high frequency in unhealthy food choices seen early in the COVID-19 pandemic improved over time.

The present study aimed to (1) examine the changes in sleep habits and dietary intake among school-aged children after the school re-opening from a 3-month closure (without school lunch) due to the COVID-19 pandemic, and (2) examine whether the changes differ between those with different temporal patterns of sleep and eating during school closure, namely, 'Very early', 'Early', 'Late' and 'Very late'. The latter patterns were characterised by later timings of wake up, breakfast and lunch. Questionnaires were answered twice by 4084 children (aged 8-15 years), themselves and/or their parents: first in June 2020 (for assessing lifestyle during school closure) and second, from July 2020 to February 2021 (for assessing lifestyle after school opening). After school re-opening, the participants' wake-up time became an hour earlier (95 % CI 1⋅0, 1⋅1) and sleep duration got 0⋅94 h shorter (95 % CI 0⋅91, 0⋅97) than during school closure. An increase in dietary intake was observed for thiamine, vitamin B6, potassium, fruits and dairy products, and a decrease was observed for sugars (as foods) and confectioneries and sweetened beverages, despite small effect sizes (Cohen's d: 0⋅20-0⋅30). Significant changes in wake-up time, sleep duration and sweetened beverage intake were observed among children with the latter temporal patterns. Thus, children wake up earlier and sleep for shorter durations after school re-opening than during school closure; however, changes in dietary intake were generally insignificant. Dietary intake among school-aged children in Japan during school closure (without school lunch) might not be worse than that during school days with universal school lunch.

BACKGROUND: The COVID-19 pandemic has further exacerbated physical inactivity, poor dietary intake and reduced mental wellbeing, contributing factors to non-communicable diseases in children. Cooking interventions are proposed as having a positive influence on children's diet quality. Motor skills have been highlighted as essential for performance of cooking skills, and this movement may contribute to wellbeing. Additionally, perceived competence is a motivator for behaviour performance and thus important for understanding intervention effectiveness. Therefore, this research aimed to assess the effectiveness of an adapted virtual theory-based cooking intervention on perceived cooking competence, perceived movement competence and wellbeing. METHODS: The effective theory-driven and co-created 'Cook Like A Boss' was adapted to a virtual five day camp-styled intervention, with 248 children across the island of Ireland participating during the pandemic. Pre- and post-intervention assessments of perceived cooking competence, perceived movement competence and wellbeing using validated measurements were completed through online surveys. Bivariate Correlations, paired samples t-tests and Hierarchical multiple regression modelling was conducted using SPSS to understand the relationships between the variables and the effect of the intervention. RESULTS: 210 participants had matched survey data and were included in analysis. Significant positive correlations were shown between perceived cooking competence, perceived movement competence and wellbeing (P < 0.05). Children's perceived cooking competence (P < 0.001, medium to large effect size), perceived movement competence (P < 0.001, small to medium effect size) and wellbeing (P = 0.013, small effect size) all significantly increased from pre to post intervention. For the Hierarchical regression, the final model explained 57% of the total variance in participants' post-intervention perceived cooking competence. Each model explained a significant amount of variance (P < 0.05). Pre-intervention perceived cooking competence, wellbeing, age and perceived movement competence were significant predictors for post-intervention perceived cooking competence in the final model. CONCLUSION: The 'Cook Like A Boss' Online intervention was an adapted virtual outreach intervention. It provides initial evidence for the associations between perceived cooking competence, perceived movement and wellbeing as well as being effective in their improvement. This research shows the potential for cooking to be used as a mechanism for targeting improvements in not only diet quality but also movement and wellbeing. TRIAL REGISTRATION: NCT05395234. ABSTRACT

Over the past two years, the world has faced the pandemic, COVID-19, and various changes. Several regulations and recommendations from the Ministry of Health of Indonesia have contributed to behavioral changes among Indonesian residents, especially in food consumption patterns. The change in food consumption patterns can be a positive change that formed due to the COVID-19 pandemic. This study aimed to examine whether the application of a Health Belief Model (HBM)-based nutrition education programme can be effectively used in changing the beliefs of adults with or without a COVID-19 history in supplement and nutrient intake. This study was a cross-sectional study involving 140 adults. This study placed 70 adults with/without a COVID-19 history into the intervention group. The intervention group participated in a nutrition education programme. The respondents were asked to fill out the questionnaire. The data were analyzed by independent and paired t-tests and Chi-square test. The result of this study showed no association between perceived susceptibility, severity, benefit, barrier, and self-efficacy, of nutrient and supplement intake with the history of COVID-19 among the respondents. However, most of the respondents in this study were low in their scores of perceivedness. Thus, it is still important for the government to increase nutrient and supplement intake education, especially in young adults aged below 25 years old.

ABSTRACT

BACKGROUND: In the wake of the severe impact of COVID-19 on the food security of the vulnerable groups in rural areas, the issue of how to achieve the Sustainable Development Goals 2 aims to "Zero Hunger" (SDG 2) and ensure the food safety of farmers has drawn unprecedented attention. Nutritional intake is generally used as an important indicator to reflect family food security. Under the background that Chinese farmers have gradually changed from the traditional diversified production mode to the specialized production of crops, the main purpose of this article is to explore what are the impact of crop specialization on farmers' nutritional intake? Could the specialization of crop production be taken as an important measure to ensure the food safety of farmers and achieve the SDG 2? METHODS: Based on the micro-survey data from 866 farmer households in China, this paper using Seemingly Unrelated Regressions model, Group Regression model and Mediating Effect model to analyze the average and heterogeneous effects of crop specialization on the nutritional intake of farmers, as well as the mediating effect of income. In addition, robustness test and endogenous treatment were performed by using alternative explanatory variables and IV-2SLS method was used to estimate the results. RESULTS: After correcting for endogenous bias, crop specialization had a significant negative impact on energy intake and fat intake of farmers at the statistical level of 5% and 1% respectively, especially for farmers in mountainous areas. Household income played a mediating effect on the effect of crop specialization on farmers' energy and fat intake, and the proportion of the masking effect was 8.43% and 8.96% respectively. In addition, household financial capital and social capital have a significant positive impact on farmers' nutritional intake. CONCLUSIONS: Crop specialization cannot guarantee the food safety of farmers in terms of nutritional intake. However, when the development trend of crop specialization is irreversible, more attention should be paid to improving the level of various livelihood capital of farmers, especially those in mountainous areas, and to continuously increasing their income to ease and ultimately eliminate the negative impact of crop specialization on farmers' nutritional intake, which finally make everyone realize the SDG 2.

Meal timing is an important consideration when assessing health. The primary outcome of this pilot study was to examine the feasibility and preliminary efficacy of early time-restricted eating (eTRE) on diet quality in a sample of college students attending a large southwestern university. Cardiometabolic indices and anthropometric measures were also obtained. The study was planned as an eight-week intervention; however, COVID-19 necessitated laboratory closures that disrupted data collection at week eight. Hence, only data obtained at week zero (baseline) and week four were viable for analysis and are presented herein. Twenty-five healthy participants were randomized to the intervention group (eTRE) which consisted of a daily 6-hour eating window or the control group (CON) which consisted of a daily 16-hour eating window. Each week, participants were permitted one day off from their respective study protocol. Seventeen participants completed all aspects of the study, and diet quality data were obtained from one additional participant (n = 18). Preliminary findings indicate that the 6-hour eTRE protocol may be beneficial for weight reduction and for reducing energy-dense foods typically consumed during the evening hours or later at night. Future research should accommodate individual preferences with regard to the initiation time of the eating window while also providing evidence-based nutrition recommendations to improve diet quality.

The objective of this study was to assess changes in body weight, body fat, food intake, and clinical risk factors during the 2020 COVID-19 pandemic (COVID group) vs. the pre-COVID period (pre-COVID group). Clinical measurements were collected and Food Frequency Questionnaires (FFQs) were administered at two time points for each group (211 days ± 114 SD). For the pre-COVID group, the data were collected before February 20, 2020. For the COVID group, the data were collected either before and after February 20, 2020, or both between February 20, 2020 and April 1, 2021, excluding a 6.5-month pandemic-related pause of hospital visits. Increases in the following outcome measures were seen in the COVID group relative to the preCOVID group: body weight (t = 3.40, p = 0.004), body fat mass (t = 2.29, p = 0.024), diastolic blood pressure (BP) (t = 2.10, p = 0.039), total cholesterol (t = 1.81, p = 0.074, marginal), and fat/oil intake (t = 2.44, p = 0.017). In contrast, there were decreases in fruit intake (t = -1.88, p = 0.064, marginal) in the COVID group compared to the preCOVID group. The COVID period relative to pre-COVID was associated with unfavorable changes in body weight and composition, food intake, and health risk factors. This appears to be the first report of in-person direct measures of changes in body weight and risk factors.

ABSTRACT

OBJECTIVES: To review the clinical management and outcomes of magnet ingestions at a large tertiary children's hospital. To determine the association of frequency of high-powered magnet ingestion with the regulation of these magnets. METHODS: Children <18 years who presented to the emergency room and were admitted to the Children's Hospital of Philadelphia for ingestion of single or multiple magnets from January 2008 to December 2020 were included. Demographics, symptoms, management, and outcomes were analyzed. The frequency of magnet ingestion was compared over 3 eras: (1) pre-ban (2008-2012), (2) intra-ban (2013-2016), and (3) post-ban (2017-2020). RESULTS: There were 167 magnet ingestions, including 99 with multiple magnets. Most patients (59%) were male and median age was 6 (interquartile range, 3-9) years. Most single magnet ingestions (86%) were discharged with outpatient monitoring, and none experienced severe outcomes. Multiple magnet ingestions led to significant morbidity including hospitalizations (68%), endoscopic procedures (48%), surgical procedures (14%), and severe outcomes (12%). Most patients (75%) were asymptomatic, however, there was a higher risk of surgery and severe complications based on the presence of symptoms ( P = 0.003). The rate of surgical intervention was higher with ≥3 magnets (31.7%) compared to 2 magnets (2.4%) ( P < 0.003). Additionally, we found an 160% increase in children with magnet ingestions in the post-ban period ( P = 0.021). CONCLUSIONS: Multiple magnet ingestion is associated with high morbidity and rate of severe outcomes. There is a relationship between public policy of magnet sale and frequency of magnet ingestion.
